Part of the Star Wars Trilogy Special edition box set from Japan. Each movie in the set comes with a beautiful insert with stills from the film and chapter stop information. The front of the insert is identical on all three films. This disc was also released separately, with identical inserts. The only difference is that the single discs have a different catalog number and feature the price on the back cover as seen below:
