Part of the Star Wars Trilogy Special edition box set from Japan. Each movie in the set comes with a beautiful insert with stills from the film and chapter stop information. The front of the insert is identical on all three films. All three discs were also available separately. The only difference is that the box-set discs have a different catalog number and don't feature the price on the back cover as seen below:
